Job Summary
The Licensed Practical Nurse is responsible for providing nursing care services and case management to clients and families by implementing nursing interventions set forth by the plan of care.
Provides direct patient care as defined in the State Nurse Practice Act and as ordered by the physician.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse, RN Case Manager and/or Director of Clinical Management.
