Key Requirements

  • Two years of experience as NP or PA
  • Graduate of accredited school of Nursing/PA program
  • Valid unrestricted NP license AZ
  • Valid unrestricted PA license AZ
  • NCCPA Board Certification
  • ANCC or AANP Board certification
