Establishes course of action by exploring options and setting goals with residents and their families to achieve the highest practicable level of self-care, independence, and well-being.
Provides medically related social services, emotional support, and addresses emotional problems, including grief related to loss or death, to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident.
Assists residents with personal shopping, inventory of belongings, securing resources from community agencies, and obtaining prosthetics/assistive devices, while coordinating with interdisciplinary teams and maintaining strong working relationships.
