Job Summary
The Field Licensed Practical Nurse provides direct patient care to hospice patients in a variety of settings and levels of care under the direct supervision of an RN.
Responsibilities include providing care to the terminally ill patient, reinforcing patient and family training and education, performing patient assessment and documentation, and participating in the comprehensive Interdisciplinary Plan of Care.
All services will be provided 24 hours a day, 7 days a week as assigned to a variety of teams and duties, including nights and weekends, with on-call rotations as assigned.
