Filos Community Services Ltd. is seeking a Senior/Assistant Senior Social Worker to lead and implement evidence-based dementia support programs in Singapore. The ideal candidate should possess 3 to 5 years of relevant experience in mental health and dementia care, with strong case management and community engagement skills
Job Summary
This role provides leadership in developing and evaluating evidence-based dementia and caregiver support programmes within the community.
The successful candidate will conduct comprehensive assessments using the Bio-Psycho-Social-Spiritual model to create individualised care plans for clients with mental health conditions.
Key responsibilities include delivering specific Care Connect programmes such as Reminiscence Connect, Cognitive Stimulation Programme, and the Eldersitter Programme.
