Job Summary
Develop individualized care plans by working closely with residents and their families to support them in achieving the highest level of independence and well-being.
Provide medically related social services to enhance residents’ physical, mental, and psychosocial health, including counseling and emotional support.
Coordinate discharge planning with interdisciplinary teams and outside agencies, and connect residents with community resources and support services.
