Job Summary
Responsible for care management and related programs within an assigned Clinical Program, functioning with collaborative implementation and evaluation of Clinical Integration Programs.

Ensures effective planning and arranging for needed services upon discharge, evaluating effectiveness by monitoring 30-day readmissions.
