Job Summary

• The Community Health Coordinator IV serves as the primary case manager for the UofL 550 Clinic, supporting people living with HIV/AIDS (PLWH/A) in accessing medical care, essential support services, and long-term retention in care.
• Responsibilities include conducting initial assessments, maintaining patient eligibility records, managing intake and enrollment, and making timely referrals to Part B and other supportive services to ensure continuity of care.
• The position requires collaboration with clinical staff and regional case management teams, leading consumer support groups, and maintaining accurate records in various systems like Provide Enterprise and EMR.

• Minimum 3 years experience with diverse populations
