Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact their medical condition, treatment, recovery, and safe transition from one care environment to another.
Confer with the Renal Patient, the Renal Patient's physician, discharge planners and social workers, and appropriate HOSPITAL staff, regarding post-discharge plans, including, but not limited to, the start of out-patient dialysis, transition to the outpatient dialysis setting, referral to nephrologists in the community, and information regarding vascular access services.
