  • Social work services include psychosocial assessments, mental health counseling, crisis intervention, financial counseling, discharge planning, health education, and the provision of material resources and linkage with community agencies.
  • Social work services are provided to patients/families to meet their medically related social and emotional needs as they impact their medical condition, treatment, recovery, and safe transition from one care environment to another.
  • Confer with the Renal Patient, the Renal Patient's physician, discharge planners and social workers, and appropriate HOSPITAL staff, regarding post-discharge plans, including, but not limited to, the start of out-patient dialysis, transition to the outpatient dialysis setting, referral to nephrologists in the community, and information regarding vascular access services.
