Job Summary

  • Manages quality initiatives and performance improvement projects by effectively interacting with management and the Compliance department.
  • Leads or directs quality and education initiatives, ensuring implementation of task forces and action plans aimed at maintaining Rochester Regional Health Home Care and Hospice as a leader in Home Care quality.
  • Supports the Orientation of new staff throughout the agency, meeting the learning needs of staff and promoting staff retention, quality health care, employee growth, and personal wellness.

Key Requirements

  • Bachelor's Degree required
  • 5 years of experience in clinical licensure area
  • Active NYS licensure as RN, PT, OT, SLP, or LMSW
  • Master's degree in healthcare field preferred
  • 3-5 years of experience leading Quality Assurance/Improvement
  • Prior management experience preferred
  • Experience with OASIS, ICD-10 coding, Home Health Star Ratings, Home Health Value Based Purchasing, and CMS Home Care and Hospice Quality Reporting Programs
  • Certification as CPHQ and COS-C desired
