Job Summary
Provide and lead social work intervention and project/event management to support the work in the Day Activity Centre/Sheltered Workshop for persons with disabilities
Case Work Management Conduct needs and intake assessments of client/family and provided social report with recommendations, to allow them to live independently and be participative in the community
Conduct case work management for cases of different risk levels
