Job Summary
Pair Team is reimagining how Medicaid and Medicare serves underserved populations by delivering whole-person care through a tech-enabled medical group.
The role involves maintaining an ongoing caseload, engaging members with Serious Mental Illness/Substance Use Disorder, homelessness, or high medical needs.
This position primarily allows for remote work, with 2-3 required on-site visits per month in the community.
