Base: $39.60 - $59.40 per hour; Bonus/Equity: Up to $10,000 sign on bonus for external candidates; Benefits: Comprehensive health, retirement, PTO, education assistance, adoption assistance and more

Key Requirements

  • Graduate from accredited Physical Therapy program
  • 2 years of Physical Therapy experience
  • Illinois or Wisconsin licensure or eligibility with supervision authorization
  • Certified Basic Life Support (BLS) or completion within 90 days
  • Valid state driver’s license and reliable automobile
