The Blended Case Manager promotes the use of community-based resources to assist individuals with intellectual, developmental disabilities, and mental health challenges.
Employees receive a $1,000 stay bonus for full-time employees paid out after orientation, 90-day probation, and 6 months of employment.
This position requires 75% of work hours to be spent out of the office in the community providing direct support.
