  • Current Arkansas Physical Therapy License
  • eligible to obtain license within 1 month
  • Bachelor's degree in Physical Therapy (prior to 2002)
  • Master's degree in Physical Therapy (prior to 2016)
  • Doctorate degree in Physical Therapy
  • Intermediate computer skills
  • pass state abuse registry checks
  • submit to TB skin test
