Job Summary

• The Primary Care Liaison serves as the strategic care coordination hub for our geriatric primary care team, supporting comprehensive care delivery for a panel of 500-600 patients in partnership with nurses and Primary Care Providers.
• You'll serve as a trusted liaison between patients, families, facility staff, and our clinical team while maintaining the highest standards of communication, documentation, and patient experience.
• Success will be measured in patient communication & responsiveness, care coordination & task management, and clinical documentation & quality.
