Job Summary
The Social Worker (Grade 2) works as a member of a multi-disciplinary team (MDT) that provides, quality patient –centered nursing care and advocate for and facilitate the involvement of individuals, their families and significant others in planning and evaluating care.
The aim is to work towards recovery and improved health outcomes through assessment, planning, implementation and evaluation of clinical interventions ensuring a holistic approach to the client, their family and carers.
