Job Summary

• The primary purpose of this position is to assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs to ensure residents’ medically related emotional and social needs are met on an individual basis.
• This role involves collaboration with administration, medical, nursing staff, and other departments to coordinate social services and provide consultation to staff and community agencies.
• The position requires maintaining confidentiality of resident care information and reporting any suspected incidents of fraud or unauthorized disclosure.
