This role requires collaboration with admitting providers, case management, and field care teams to assess clinical information and confirm patient eligibility for home-based acute care.
The position offers a full-time schedule with self-scheduling capabilities and supports patients across the South/West Region including Newton-Wellesley Hospital, Brigham and Women's Hospital, and Brigham and Women's Faulkner Hospital.
