Key Requirements

• RN, LPN, MSW, OT, PT, or SLP licensure
• 3–4 years acute care clinical practice preferred
• 1–2 years home care direct patient experience preferred
• 1–2 years intake or sales experience preferred
• Current Illinois registration or licensure preferred
• Valid driver’s license and reliable transportation
