Job Summary

  • The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services.

Key Requirements

  • Baccalaureate Degree, ADN, or Diploma RN Degree
  • 3 years RN experience
  • current BLS certification
  • Valid Arkansas Registered Nursing License
  • Certification in Case Management (CCM, ANCC) preferred
  • BSN preferred
  • Care Management experience in Ambulatory setting
