Job Summary

  • The Department of Health and Human Services (DHHS) is looking for outstanding Service Coordinators within the Division of Developmental Disabilities (DDD) to make a difference in people's lives.
  • In this position, you will serve as a case manager for people who have been determined eligible for a Waiver, coordinating and overseeing service delivery, conducting assessments, developing plans, providing referrals, and monitoring services.
