The position involves conducting transitional care outreach to patients within 48 hours of hospital discharge and assisting with effective coordination of services internally and externally.

Key Requirements
Baccalaureate Degree, ADN, or Diploma RN Degree
3 years RN experience
current BLS certification
Valid Arkansas Registered Nursing License
Certification in Case Management (CCM, ANCC) or obtain within 1 year
BSN preferred
Care Management experience in Ambulatory setting
