Job Summary
The role involves advocating for patients living with kidney disease to achieve maximum social functioning and psychological adjustment.
You will conduct required patient assessments, complete the KDQOL, and coordinate financial and social resources in accordance with CMS regulations.
As part of an interdisciplinary team, you will collaborate with medical directors and participate in Quality Assessment and Performance Improvement activities.
