The RN Care Manager provides longitudinal care management services to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s) and complex health needs.
Responsibilities include conducting transitional care outreach to patients within 48 hours of hospital discharge and assisting with effective coordination of services internally and externally.

Key Requirements
Baccalaureate Degree, ADN, or Diploma RN Degree
3 years RN experience
Current BLS certification
Valid Arkansas Registered Nursing License
Certification in Case Management (CCM, ANCC) preferred
