Job Summary

• The Certified Mental Health Professional utilizes clinical expertise to perform psychosocial assessment and provides quality behavioral health services including being responsible for treatment plan development, coordination, and the delivery of psychotherapy to patients and families.
• This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients.
