As a Care Navigator, you will complete comprehensive psychosocial assessments with a focus on the patient’s requirements as they transition to the next level of care.
Identify and navigate patient testing and treatment to reduce barriers to patient discharge and prevent delays in patient care; communicate barriers to leadership for resolution and trending.

BSW or MSW or Associate's degree in Nursing
3 years experience for SW
1-3 years experience for RN
Prior Case Management experience preferred
Active Maryland License
Active AHA BLS certification
Prior Cerner EMR and All Scripts experience preferred
