Job Summary

The Case Manager works with physicians and multidisciplinary team members to develop a plan of care for assigned patients, identifies and resolves barriers, and is actively involved in discharge planning.
This position must integrate company values into daily practice and is PRN, as needed to support full time employee vacation coverage, weekends, and occasional weekdays.
