  • The role requires tracking and monitoring facility effective utilization of PDPM tools including Physician Diagnosis Verification and admission/discharge data.
  • This position involves assessing health status for new admissions and ensuring accurate electronic submission of all MDS assessments.

Key Requirements

  • Bachelor of Science Degree in Nursing preferred
  • 3 years of supervisory experience preferred
  • Long Term Care Experience Preferred
  • Good standing with State Board of Nursing
