The incumbent must assist residents in achieving independence while providing support for anxieties, stress, and coping with physical disabilities or illness.
Key responsibilities include coordinating discharge planning, obtaining community resources, and maintaining strict confidentiality of all resident care information.

Key Requirements
Bachelor's Degree in Social Work (BSW) required
One year supervised social work experience in healthcare setting
Social work licensure (LSW, LMSW, LCSW) if state requires
Completion of mandatory in-service trainings including HIPAA
Must meet federal qualification standards for skilled nursing facilities
