The ideal candidate will have a Master's in Social Work, be license-eligible, and have experience working with individuals with mental illness or developmental disabilities

Job Summary

  • This position serves as the primary discharge planner for individuals who have been hospitalized for an extended period.
  • This role will also require the ability to link individuals to appreciate services and resources in the community, complete assessments and referrals, and work collaboratively from a multi-disciplinary recovery perspective.
