Job Summary
Provides oversight on facilities’ weekly PDPM meetings and ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change.
Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission according to regulatory time frames and maintains thorough documentation as mandated by federal and state standards.
Assesses resident health status, communicates level of care, schedules and conducts resident care conferences, and assists in formulating and revising individualized care plans.
