Job Summary

• The Physical Therapist will follow appropriate practices, to perform evaluations, timely re-evaluations and OASIS assessments, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability.
• Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs and actively participates in multi-disciplinary team meetings, as assigned.
• Prepares clinical and progress notes that support skilled intervention and assesses outcomes of care, plans for discharge of patient, and carries out treatment plan effectively.
