Job Summary

  • Provides healthcare in primary care, acute care, critical care and long-term health care settings, participating in the medical and surgical care of solid organ transplant patients.
  • Responsible for the development, promotion and coordination of a multidisciplinary plan of care from referral through discharge to assure adequate health maintenance and long term care of the transplant patient.
