The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with their assessment and care plan.
Employees must report all changes in resident conditions, accidents, and incidents to the Health and Wellness Director or Executive Director as soon as practical.
The role requires assisting residents with personal care tasks such as bathing, dressing, feeding, and mobility while maintaining strict confidentiality of protected health information.

Skills & Requirements
Must-have
Assist residents with daily living activities
Monitor and report changes in resident condition
Maintain confidentiality of protected health information
Perform lifting and moving of residents up to 25 pounds
Complete documentation for care plans and incidents

Create warm and positive atmosphere for residents
Cooperate with inter-departmental personnel
Participate in regular nursing reports
Demonstrate ability to solve practical problems
Key Requirements
High school education or equivalent
Proficiency in Microsoft Suite products
Training in specific care tasks performed
Ability to read and interpret technical procedures
