Job Summary
Assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs to meet the medically related emotional and social needs of the resident.
Coordinate social service activities with other departments and assist residents in achieving the highest practicable level of self-care, independence, and well-being.
Provide information on financial assistance programs and assist residents and families with anxieties and stress related to illness and institutionalization.
