Job Summary

  • The Complex Care Coordinator is responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience.
  • This position is onsite 3-4x weekly and a majority of the duties include telephone contact with patients, some weeks are very limited in person contact.
