Job Summary

  • This position is responsible for leading team meetings and work groups of families of children and youth with special health care needs (CYSHCN) to work on the development and maintenance of effective programs for CYSHCN.
  • The position trains, assists and provides guidance to staff on family related activities including, but not limited to, family perspectives, family centered care, care coordination, transition planning, medical home, and educational/community resources.
