Job Summary

  • The primary purpose of this role is to assist in planning, developing, organizing, implementing, evaluating, and directing facility's social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
  • Key duties include assisting residents in achieving the highest practicable level of self-care and well-being, providing consultation to staff and community agencies, and coordinating social service activities with other departments.
  • Health benefits are offered to Full-Time Employees, including Medical, Dental, Vision, and a 401K, with a pay wage of $23/HR DOE.
