The position requires coordinating social service activities with other departments and obtaining resources from community agencies to ensure the highest level of well-being for each resident.
Employees must maintain strict confidentiality of all resident care information while providing consultation to staff and families regarding social treatment plans and institutional life adjustments.

Key Requirements
