Job Summary
The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.
Case Manager RNs are responsible for advocating for the patient and family throughout the entire episode of care, monitoring patient’s progression intervening as needed, and coordinating all aspects of discharge planning.
