Job Summary

  • You will provide person-centred care coordination for clients living with complex aged and healthcare needs, whilst also utilising discipline specific skills to support each client to improve their health and wellbeing.
  • This position is under the Health Independence Program (HIP) and will be part of the HIP Complex Care and Chronic Disease Team.
  • Full time employment comes with a monthly Accrued Day Off (additional day off per month!)
