Adventist HealthCare is seeking a Per Diem Physical Therapist for their Acute Care Rehab department at Shady Grove Medical Center. The ideal candidate will perform patient evaluations and treatment, manage care plans, and collaborate with a multidisciplinary team, all while embodying the organization's mission of holistic healing
Job Summary
Performs an initial examination and evaluation to establish a impairment/diagnosis, prognosis and plan of care prior to intervention for all persons served with basic rehabilitation needs in acute care, outpatient or inpatient rehabilitation settings.
Communicates, coordinates and documents all aspects of patient/client management including the results of the initial examination and evaluation, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, changes in patient/client status relative to the interventions, reexamination, and discharge/discontinuation of intervention and other patient/client management activities with members of the care team as indicated.
