Key Requirements

  • Doctoral degree from accredited school of medicine or osteopathic medicine
  • Three years post-residency experience evaluating complex patients
  • Current unrestricted Arizona medical license
  • Board certification from ABFM, ABIM, or ABEM
  • Completed approved US Residency/Fellowship in Family Medicine, Internal Medicine, or Emergency Medicine
  • Current DEA certification and dispensing license
  • Current CPR certification
