Job Summary

  • Identify patients who are vulnerable, high risk for readmission within 30 days of discharge, and/or high ED utilizers requiring additional support and connections to community resources.
  • Act as a patient advocate by identifying and connecting patients to the most appropriate community resources.

Key Requirements

  • Registered Nurse (RN) or Master of Social Work (MSW)
  • Minimum 1-3 years clinical experience (RN)
  • Minimum 3 years clinical experience (SW)
  • Licensed Registered Nurse (Maryland)
  • Licensed Social Worker (LMSW or LCSW or LCSW-C, Maryland)
  • BLS certification
