This role is responsible for assisting in the planning, developing, and directing of social service programs to meet the medically related emotional and social needs of residents.
The position requires coordinating social service activities with other departments and obtaining resources from community agencies to ensure the highest practicable level of well-being for each resident.
Employees must maintain strict confidentiality of all resident care information and report any suspected incidents of fraud or unauthorized disclosure of protected health information.

Must-have
Assist in developing social service programs
Coordinate discharge planning with agencies
Maintain confidentiality of protected health information
Provide consultation on Medicare/Medicaid assistance
Document social service progress notes regularly
Nice-to-have
Prior long-term care experience preferred
Develop good working rapport with community agencies
Participate in orientation and in-service training
Assist residents with anxiety and stress coping
Key Requirements
Bachelor's Degree in Social Work (BSW) required
One year supervised social work experience in healthcare setting
State social work licensure (LSW, LMSW, LCSW) if applicable
Completion of mandatory in-service trainings including HIPAA
