This role involves assessing Veteran needs, facilitating access to healthcare services and VA benefits, and supporting safe transitions of care across hospital, VA, and community systems.
The coordinator also contributes to system improvements by sharing best practices, developing educational materials, and assisting with staff and clinician education.

Key Requirements

Current AAMA or RMA certification preferred
CHW and Motivational Interviewing certifications preferred
Graduate of LPN or accredited RN program preferred
Completion of specified military medical training programs
