Job Summary

• The LVN Case Manager will provide daily care coordination with concurrent review for patients in the hospital or Skilled Nursing Facilities (SNF) telephonically.
• Works as part of an interdisciplinary care team coordinating care and collaborating with social work, ambulatory case management, Regional Medical Directors, Hospitalists, the hospital nursing personnel, and the physician in the clinic.

• Graduation from accredited LVN program
• Current valid LVN License
• Three years public health/acute care/case management/in-home care
• Minimum 2 years managed care case management
• Valid BLS certification or higher
