The Transitions of Care (TOC) Manager is responsible for designing, implementing, and overseeing pharmacy services that support safe, effective, and seamless transitions across the continuum of care.
Develops workflows, protocols, and policies that support efficient and compliant TOC operations.
Responsible for supervising and performing all pharmacy operations in accordance with hospital policies and procedures, ethical and professional practices, accreditation standards, State and Federal requirements and National Patient Safety Goals regarding medication management.

Key Requirements
Bachelor's degree in pharmacy or Doctorate in Pharmacy
Minimum of 3 years of job-related experience
PGY-1/PGY-2 ASHP Residency in administration
Management experience required
Current Kentucky and Indiana Pharmacist License or eligible
Must obtain Kentucky and Indiana Pharmacist License within 1 year
