Job Summary
The Social Worker develops discharge plans through coordination of community resources and collaboration with the patient, Treatment Team, Clinical Social Worker, Managing Care Organizations (MCO), and other agencies.
This position will manage a caseload of psychiatric patients, some of whom may be dually diagnosed, and provides case management services and psychoeducation aimed at skill building for the patient.
